The problem it solves
Texas licenses home care agencies through Health and Human Services, and HHSC surveys them unannounced, whether clients pay privately or not. A survey does not ask whether your caregivers are kind. It asks you to prove, on paper and on the day, that your agency does what its policies say.
That is where small agencies get caught. Incidents get handled by phone and never written into a log. Training happens after the first shift instead of before it. A policy manual bought years ago still describes an agency that no longer exists. None of that means the care was poor. All of it gets cited.

How the system works
Staff follow written procedures and document what they did. Supervisors review that documentation and sign it. An administrator reads across incidents, complaints and training for patterns, and owns the corrective action. In a two-person office one person holds two of those roles, and that is fine as long as the roles are written down and the reviews still happen.
